Form Name: Student Profile and Waiver 
Description: Student Profile forms are required to be on file and updated yearly for every member family.  Ther is a $40 annual (non refundable) membership fee due with your initial registration (and yearly on your anniversary month).  We will also need a copy of your medical insurance card. 
   
Form Name: Open Gym/Birthday Party/Special Event Participation Waiver 
Description: All gym guests must have a Special Event Participation waiver on file.  These forms will be vaild for one year.
